Woodlands Health invites a dedicated Medical Social Worker to join our patient-centered care team at Woodlands Hospital in Singapore. This role focuses on holistic medical social work that supports recovery, resilience, and community care. You will collaborate with clinicians to assess psychosocial needs, advocate for patients and families, and coordinate access to social services and community resources. In this position you will engage with patients and families across the care continuum—admission, treatment, discharge, and post-discharge support. You will conduct psychosocial assessments, identify barriers to care, facilitate care planning, and assist with housing, financial assistance, and community supports. You will participate in multidisciplinary rounds, contribute to discharge planning, and ensure smooth transitions to home or community-based care. You will maintain accurate documentation in the EMR and uphold privacy and confidentiality in accordance with regulatory standards. Responsibility
- Conduct comprehensive psychosocial assessments for inpatients and families to inform care planning.
- Develop and implement individualized care plans, including discharge planning and community resource linkage.
- Coordinate with medical teams, therapists, and social services to address barriers to recovery and promote patient-centered outcomes.
- Provide crisis intervention, counseling, and ongoing psychosocial support during hospital stays and transitions.
- Advocate for patient rights and preferences, ensuring culturally appropriate care and informed consent.
- Connect patients with financial assistance, housing options, transportation, and social support programs.
- Document assessments, plans, and progress accurately in the electronic medical record.
Qualification
- Bachelor's or Master's degree in Social Work; registered as a Social Worker with the Singapore Social Work Registration Board where applicable.
- Minimum 2 years of hospital or community-based social work experience; clinical experience in medical settings preferred.
- Strong knowledge of discharge planning, care coordination, and resource navigation.
- Excellent communication, empathy, and ability to work collaboratively with multidisciplinary teams.
- Ability to manage confidential information and navigate sensitive family dynamics.
- Proficiency with electronic medical records and social work documentation; fluent in English; additional languages welcome.
