Join PAP Community Foundation as a full-time Occupational Therapist and play a pivotal role in enhancing the quality of life for seniors in the community. Based in the Eunos area, you will work with a multidisciplinary team to deliver compassionate, client-centered rehabilitation and enable seniors to maintain independence and meaningful daily activities.
As a trusted member of our community health program, you will assess functional abilities, design individualized care plans, and advocate for home and community supports that promote safety, dignity, and participation. You will collaborate with caregivers and family members, healthcare professionals, and community partners to implement sustainable interventions that align with seniors goals and the Foundation’s mission to nurture happy, healthy seniors.

Responsibility
- Conduct comprehensive OT assessments for seniors in community-based settings and develop person-centered care plans.
- Design and adapt activity programs to promote independence, mobility, cognition, and quality of life.
- Assess living environments and recommend safe home modifications or assistive devices; train clients and caregivers on usage.
- Coordinate with multidisciplinary teams to implement rehabilitation goals in the community or center.
- Provide caregiver education and support, including training on transfers, equipment, safety, and activity planning.
- Document progress, review care plans regularly, and adjust interventions based on outcomes and feedback.
- Deliver home visits and community workshops to raise awareness about fall prevention, energy conservation, and daily living skills.
- Advocate for seniors rights and connect families with community resources and programs.
Qualification
- Bachelor's or Master's degree in Occupational Therapy from an accredited institution.
- Registered with Singapore's Allied Health Professions Council (AHPC) and eligible to practice as an Occupational Therapist in Singapore.
- Valid Practicing Certificate; current professional indemnity coverage as applicable.
- Minimum 1 year of relevant experience in elderly care, community rehab, or related settings.
- Strong clinical assessment, problem-solving, and care-planning abilities.
- Excellent communication and teamwork skills to collaborate with clients, families, and multidisciplinary teams.
- Experience with assistive devices, home modifications, and outcome measurement tools.
- Genuine passion for aging, person-centered care, and community empowerment.
