Join Assisi Hospice as a Senior / Medical Social Worker and play a pivotal role in delivering compassionate, patient-centered psychosocial care for individuals facing life-limiting illness and their families. Working within a multidisciplinary team, you will conduct psychosocial and financial assessments, develop person-centered care plans, coordinate social work interventions, and provide counseling to help families navigate illness, bereavement, and transition planning. Your expertise will contribute to enhanced quality of life, dignity, and informed decision-making for patients and their loved ones.
As a core member of our palliative care team, you will advocate for patient rights, facilitate discharge planning, and connect clients with community resources and financial assistance schemes. You will lead and participate in case conferences, coordinate home-based services and visits, and document progress in our electronic medical records. This role also involves bereavement support and caregiver education, ensuring a continuum of care that extends from hospital to home and community.

Responsibility
- Conduct comprehensive psychosocial assessments for patients and families to identify needs, risks, and goals of care.
- Develop and implement individualized care plans in collaboration with the patient, family, and multidisciplinary team.
- Provide counseling, crisis intervention, and bereavement support to patients and caregivers.
- Coordinate discharge planning and home-based or community-based services to enable care at home.
- Facilitate access to financial assistance, subsidies, and social services; assist with applications and documentation.
- Advocate for patient rights, cultural sensitivity, and ethical practice in all interactions.
- Maintain accurate documentation of assessments, plans, and progress in the electronic medical records system.
- Collaborate with physicians, nurses, spiritual care providers, and external agencies to ensure holistic, patient-centered care.
Qualification
- Bachelor's or Master’s degree in Social Work with eligibility for practice in Singapore; professional registration preferred.
- Minimum 3 years of medical social work or palliative care experience.
- Strong skills in psychosocial assessment, counseling, and crisis intervention.
- Knowledge of Singapore social services, subsidies, and community resources; ability to navigate funding applications.
- Excellent communication, interpersonal, and collaboration skills for multidisciplinary teams.
- Proficiency in documentation and case management systems; high attention to privacy and ethics.
- Ability to manage complex cases, prioritize workload, and respond effectively to sensitive situations.
- Cultural sensitivity and a patient-centered, empathetic approach to care.
