HopeBridge Community Services is seeking a compassionate and results-driven Case Manager to join our Seattle team. This role focuses on helping individuals and families access stable housing, benefits, and community supports while maintaining dignity and empowerment.
As a Case Manager, you will be integral to coordinating services, advancing clients toward measurable outcomes, and upholding the highest standards of ethical care. This is a full-time role with opportunities for professional growth, supervision, and ongoing training.
Responsibility
- Conduct comprehensive client assessments to identify needs, assets, and risks.
- Develop and monitor individualized service plans with clear outcomes and timelines.
- Coordinate access to housing, benefits, healthcare, legal aid, and educational services.
- Provide crisis intervention, supportive counseling, and referrals as needed.
- Maintain accurate documentation and case notes in the electronic records system.
- Advocate for clients with community partners and service providers to remove barriers.
- Collaborate with an interdisciplinary team to ensure holistic care and compliance with funding requirements.
Qualification
- Bachelor's degree in Social Work or related field; MSW preferred.
- Minimum of 2 years of case management or social services experience.
- Strong knowledge of community resources, housing programs, benefits, and child welfare services.
- Excellent communication, listening, and trauma-informed approaches.
- Ability to work with diverse populations; bilingual Spanish preferred.
- Proficiency with electronic case management systems and data entry.
- Valid driver’s license and reliable transportation may be required.
