Hope & Harbor Community Services is seeking a dedicated Case Manager to support individuals and families in the Seattle area. You will assess client needs, create responsive service plans, and connect clients with essential resources to promote stability, safety, and independence.

Responsibility
- Conduct comprehensive intake and assessments to determine client eligibility and service needs.
- Develop, implement, and monitor individualized service plans in collaboration with clients and family members.
- Coordinate with housing, employment, health, and social support resources to secure needed services.
- Document case notes, progress, and outcomes in our EMR with accuracy and timeliness.
- Provide crisis intervention and de-escalation support when required, following safety protocols.
- Advocate on behalf of clients to access benefits, housing, and community programs.
- Collaborate with supervisors, clinicians, and community partners to ensure continuity of care.
- Maintain compliance with privacy regulations and organizational policies.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field; Master's preferred.
- Licensed Social Worker (LSW) or eligible for licensure in Washington state is a plus.
- Minimum 2 years of experience in case management or social services, ideally in a nonprofit setting.
- Strong knowledge of local community resources in Seattle and King County.
- Trauma-informed, client-centered approach with excellent communication and documentation skills.
- Proficiency with case management software and Microsoft Office; ability to learn new systems quickly.
- Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
