BrightPath Community Services is a mission-driven nonprofit dedicated to empowering families and individuals to achieve stability and self-sufficiency. We are seeking a compassionate, detail-oriented Social Services Case Manager to provide holistic support, conduct comprehensive needs assessments, coordinate community resources, and advocate for clients within a trauma-informed framework. In this role you will work with diverse populations, including families experiencing housing insecurity, unemployment, mental health challenges, and domestic violence, to remove barriers to service access and promote sustainable outcomes.
Responsibility
- Conduct client outreach and intake assessments to determine eligibility and needs;
- Develop individualized service plans and coordinate short- and long-term services with community partners;
- Provide crisis intervention and safety planning as needed;
- Advocate for clients with housing providers, healthcare systems, schools, and social service agencies;
- Maintain accurate case notes, caseload management, and timely documentation in the CRM;
- Monitor progress, review service plans, and adjust as necessary to achieve goals;
- Collaborate with multidisciplinary teams to ensure client-centered care and culturally competent services;
- Participate in community outreach and resource development to expand access to programs.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field (MSW preferred);
- Minimum of 2 years of case management or direct service experience in a social services setting;
- Strong knowledge of community resources, housing, healthcare, and financial assistance programs;
- Excellent communication, listening, and documentation skills; ability to build trust with diverse populations;
- Proficiency in data entry and case management software; strong organizational and time-management skills;
- Crisis intervention training or demonstrated ability to respond to safety concerns;
- Commitment to equity, inclusivity, and culturally competent practice.
