Responsibility

  • Conduct comprehensive client intake assessments and develop individualized service plans to address housing stability, financial support, healthcare access, and safety concerns.
  • Coordinate with housing programs, shelter services, public benefits, healthcare providers, and child welfare agencies to secure resources for clients.
  • Provide ongoing case management, crisis intervention, and referrals, tracking progress and adjusting plans as needs evolve.

Qualification

  • Bachelor’s degree in Social Work, Sociology, Psychology, or a related field; Master’s degree preferred.
  • Current licensure or eligibility for licensure (e.g., LBSW, LMSW, LCSW) in New York or willingness to obtain within specified timeframe.
  • Minimum 2 years of direct service experience in case management or social services roles.
