Responsibility
- Conduct comprehensive client intake assessments and develop individualized service plans to address housing stability, financial support, healthcare access, and safety concerns.
- Coordinate with housing programs, shelter services, public benefits, healthcare providers, and child welfare agencies to secure resources for clients.
- Provide ongoing case management, crisis intervention, and referrals, tracking progress and adjusting plans as needs evolve.
- Advocate for clients with schools, landlords, employers, and community partners to remove barriers and promote self-sufficiency.
- Document case notes, progress, and outcomes in electronic records and ensure compliance with privacy and regulatory requirements.
- Deliver workshops and outreach activities to connect clients with available programs and foster community resilience.
- Apply traumaâinformed and culturally responsive practices to all interactions, maintaining client dignity and confidentiality.
Qualification
- Bachelorâs degree in Social Work, Sociology, Psychology, or a related field; Masterâs degree preferred.
- Current licensure or eligibility for licensure (e.g., LBSW, LMSW, LCSW) in New York or willingness to obtain within specified timeframe.
- Minimum 2 years of direct service experience in case management or social services roles.
- Strong knowledge of housing policy, public benefits, healthcare access, and community resources.
- Excellent communication, empathy, and crisis intervention skills with a culturally diverse client base.
- Proficiency with case management software, data entry, and reporting tools.
- Ability to work collaboratively in a multidisciplinary team and manage priorities in a fast-paced environment.
- Spanish or other language proficiency is a plus.
