Join BeaconBridge Community Services as a Licensed Social Worker in Boston, MA. Responsibility
- Perform comprehensive assessments to determine clients' needs, strengths, and goals, and develop individualized service plans.
- Provide evidence-based counseling and crisis intervention to individuals, families, and groups.
- Coordinate referrals to housing assistance, benefits programs, mental health and substance use services, and legal supports.
- Advocate for clients with schools, healthcare systems, and social service agencies, ensuring timely access to resources.
- Maintain meticulous case notes and documentation in our EMR system, ensuring regulatory compliance and data privacy.
- Monitor progress, reassess plans regularly, and adjust interventions to optimize outcomes.
- Collaborate with multidisciplinary teams to deliver holistic, client-centered care and coordinate with community partners.
Qualification
- Master of Social Work (MSW) from an accredited program; current LICSW/LCSW/LMSW license in Massachusetts or eligible to obtain licensure within the next 6 months.
- Minimum of 2 years of clinical or case management experience in health, child welfare, or community services.
- Strong knowledge of community resources in the Boston area and ability to make informed referrals.
- Proficiency with electronic medical records (EMR) and case management software; excellent documentation skills.
- Excellent interpersonal, crisis intervention, and de-escalation skills; culturally responsive communication.
- Commitment to equity, inclusion, and anti-racism practices; ability to work with diverse populations.
- Valid driver's license and reliable transportation for field visits is preferred or required; willingness to travel within service area.
