Hope & Home Community Services is hiring a compassionate Case Manager to support individuals and families in San Francisco. In this role, you will assess needs, develop person-centered service plans, coordinate housing, healthcare, and social supports, and advocate to remove barriers to stability and independence.

Responsibility
- Conduct thorough client assessments and risk screening; document findings in the electronic case management system.
- Develop and monitor individualized service plans with clear goals and measurable outcomes.
- Coordinate housing, medical, mental health, and social supports through partnerships with community organizations.
- Provide crisis intervention and de-escalation as needed, applying trauma-informed care principles.
- Advocate for clients with agencies, employers, and educational institutions to improve access to services.
- Maintain accurate, timely documentation and case notes to ensure continuity of care.
- Collaborate with interdisciplinary teams to review progress and adjust plans as necessary.
- Participate in supervision, trainings, and professional development to uphold service quality.
Qualification
- Bachelor's degree in social work, psychology, sociology, or a related field; Master’s preferred; LCSW or equivalent credential encouraged.
- Minimum 2 years of case management or social services experience, preferably with diverse populations.
- Strong understanding of trauma-informed care and client-centered approaches.
- Excellent communication, active listening, and negotiation skills.
- Proficiency with electronic case management systems and meticulous documentation habits.
- Ability to work independently and as part of a collaborative team; strong organizational skills.
- Fluency in English; bilingual English/Spanish a plus; commitment to equity and inclusion.
