Job description

HopeBridge Community Services is hiring a dedicated Social Work Case Manager to support families and individuals in navigating complex systems and accessing essential services. This full-time role combines clinical judgment with hands-on advocacy, ensuring equitable access to resources, safe housing, healthcare, and supportive community programs.

As part of our multidisciplinary team in New York, you will build trusting relationships, develop person-centered plans, and monitor progress to achieve measurable outcomes.

Responsibility

  • Conduct comprehensive biopsychosocial assessments and intake for new clients.
  • Develop and implement individualized case plans with SMART goals.
  • Coordinate housing, health care, child welfare, and social supports across agencies.
  • Provide crisis intervention, risk assessment, and safety planning.
  • Document case notes, maintain compliance with policy and confidentiality, and prepare reports for funders.

Qualification

  • Excellent interpersonal, de-escalation, and crisis intervention skills.
