HopeBridge Community Services is hiring a dedicated Social Work Case Manager to support families and individuals in navigating complex systems and accessing essential services. This full-time role combines clinical judgment with hands-on advocacy, ensuring equitable access to resources, safe housing, healthcare, and supportive community programs.
As part of our multidisciplinary team in New York, you will build trusting relationships, develop person-centered plans, and monitor progress to achieve measurable outcomes.
Responsibility
- Conduct comprehensive biopsychosocial assessments and intake for new clients.
- Develop and implement individualized case plans with SMART goals.
- Coordinate housing, health care, child welfare, and social supports across agencies.
- Provide crisis intervention, risk assessment, and safety planning.
- Document case notes, maintain compliance with policy and confidentiality, and prepare reports for funders.
- Collaborate with supervisors, clinicians, and community partners to reduce barriers and promote client engagement.
- Advocate for client rights, connect to financial assistance, and support transition to community resources.
Qualification
- Master’s degree in Social Work (MSW) or related field; active LMSW/LCSW license preferred.
- Minimum 2-3 years of social work or case management experience in health care, housing, or child welfare.
- Strong knowledge of NYC community resources and referral networks.
- Excellent interpersonal, de-escalation, and crisis intervention skills.
- Experience with electronic health records (EHR) and case management software; proficient documentation.
- Cultural humility and commitment to trauma-informed care and equity.
- Ability to manage complex cases, prioritize tasks, and work collaboratively in a fast-paced environment.
