BrightPath Community Services is seeking a dedicated Case Manager to join our Seattle-based social services team. You will assess client needs, develop person-centered service plans, coordinate resources, and advocate for client rights to promote stability and independence.

Responsibility
- Conduct comprehensive client assessments and risk screenings to determine eligibility and service needs.
- Develop individualized service plans and coordinate referrals to housing, healthcare, education, employment, and financial resources.
- Monitor progress, maintain accurate case notes, and ensure compliance with organizational policies and funding requirements.
- Advocate for clients, connect them with community resources, and help navigate public benefits.
- Provide crisis assessment and de-escalation support when needed; implement safety planning.
- Collaborate with multidisciplinary teams, social workers, counselors, and external partners to promote holistic care.
- Document outcomes, prepare reports, and participate in quality improvement initiatives.
Qualification
- Bachelor's degree in social work, psychology, sociology, or related field; MSW is preferred.
- Minimum 2 years of case management or frontline social services experience.
- Strong knowledge of community resources, housing programs, benefits, healthcare access, and employment supports.
- Excellent communication, active listening, and cultural humility; ability to build rapport with diverse clients.
- Exceptional organization, documentation, and time-management skills; proficiency with case management software.
- Ability to manage crisis situations and maintain professional boundaries; crisis intervention training is a plus.
- Valid driver's license and reliable transportation preferred; flexible schedule to meet client needs.
