Join HopeBridge Community Services as a dedicated Case Manager in Chicago, guiding families and individuals through essential social services. This role focuses on empowerment, access to resources, and durable outcomes that improve lives.
As a key member of our outreach team, you will partner with clients, families, and community organizations to develop personalized service plans, connect them with housing, financial assistance, healthcare, and counseling services, and advocate for equitable access to resources.
Responsibility
- Assess clients' needs and develop individualized service plans
- Coordinate with housing, healthcare, and financial assistance providers
- Advocate for clients to ensure timely access to services
- Monitor progress and adjust plans to achieve measurable outcomes
- Maintain accurate case notes and documentation in our case management system
- Provide crisis intervention and short-term support as needed
- Collaborate with inter-agency teams to maximize community resources
- Ensure compliance with program guidelines and legal requirements
Qualification
- Bachelor's degree in Social Work or a related field
- Licensed Social Worker (LSW) or equivalent certification, or willingness to obtain within 12 months
- Minimum 2 years of experience in social services or case management
- Strong communication, empathy, and cultural competence
- Ability to assess risk, prioritize needs, and manage crisis situations
- Familiarity with community resources and referral processes
- Proficiency with case management software and data entry
- Valid driver's license and reliable transportation
