Responsibility

  • Conduct comprehensive medication therapy management (MTM) and pharmacotherapy consults to optimize patient outcomes.
  • Perform medication reconciliation and drug utilization review to ensure safety, adherence, and efficacy.
  • Collaborate with physicians, nurses, and care teams to optimize treatment plans and implement formulary guidelines.
  • Provide patient counseling on medications, including dosing, adverse effects, interactions, and adherence strategies.
  • Participate in clinical rounds, contribute to care plans, and document interventions in the EHR.
  • Lead medication safety initiatives, including error reporting, root-cause analysis, and process improvements.
  • Support transitions of care and medication reconciliation across settings to reduce readmissions.

Qualification

  • PharmD degree or equivalent; active pharmacist license in California (CA Pharmacist License).
  • Minimum of 2+ years of clinical pharmacy experience in hospital or health-system setting preferred.
  • Deep knowledge of pharmacotherapy, drug interactions, contraindications, adverse event management, and evidence-based guidelines.
  • Proficiency with EHRs and medication management software; experience with formulary management and ICU/med-surg workflows.
  • Strong communication, empathy, and teamwork; ability to educate patients and collaborate with cross-functional teams.
  • Ability to work in a fast-paced environment, make time-sensitive clinical decisions, and prioritize safety.
  • Active BLS/CPR certification preferred; ongoing participation in continuing education.
