Responsibility
- Provide comprehensive medication therapy management to patients, ensuring optimal pharmacotherapy and safety.
- Lead medication reconciliation in transitions of care and prevent adverse drug events.
- Collaborate with physicians, nurses, and care teams to optimize therapy plans.
- Counsel patients on medication use, potential side effects, and adherence strategies.
- Manage immunization services and educate patients on vaccination opportunities (where licensed).
- Oversee formulary adherence, dosing accuracy, and safe dispensing processes.
- Utilize electronic health records (EHR) and pharmacy systems to document clinically relevant information and outcomes.
Qualification
- Licensed Pharmacist in California (active, no sanctions).
- Doctor of Pharmacy (PharmD) or equivalent degree; 1+ year of clinical experience preferred.
- Strong knowledge of pharmacokinetics, drug interactions, and evidence-based therapeutics.
- Excellent communication and patient counseling skills; ability to educate diverse populations.
- Experience with MTM programs, immunizations, and clinical workflow optimization.
- Proficiency with EHR systems (Epic or similar) and pharmacy management software.
- Commitment to patient safety, HIPAA compliance, and continuous quality improvement.
