Responsibility

  • Provide comprehensive medication therapy management for inpatients and selected outpatients
  • Collaborate with physicians, nurses, and other clinicians to optimize pharmacotherapy
  • Lead daily clinical rounds and offer actionable recommendations for therapy optimization
  • Ensure safe and accurate dispensing, medication reconciliation, and drug information support
  • Participate in formulary management, cost containment, and therapeutic substitutions
  • Educate patients and caregivers on medication use, potential side effects, and adherence strategies
  • Identify and resolve adverse drug events; participate in pharmacovigilance initiatives
  • Support quality improvement, regulatory compliance, and interdepartmental projects

Qualification

  • Doctor of Pharmacy (PharmD) from an ACPE-accredited program
  • Active Massachusetts pharmacist license or eligibility to obtain licensure
  • Minimum 2 years of hospital or health-system pharmacy experience
  • Strong clinical knowledge across therapeutics and medication safety
  • Excellent communication, collaboration, and leadership skills
  • Proficiency with EMR/e-prescribing systems and MTM documentation
  • Board Certification in Pharmacotherapy (BCPS) or related certification is a plus
  • Commitment to ongoing professional development and patient-centered care
