Responsibility
- Provide comprehensive medication therapy management for inpatients and selected outpatients
- Collaborate with physicians, nurses, and other clinicians to optimize pharmacotherapy
- Lead daily clinical rounds and offer actionable recommendations for therapy optimization
- Ensure safe and accurate dispensing, medication reconciliation, and drug information support
- Participate in formulary management, cost containment, and therapeutic substitutions
- Educate patients and caregivers on medication use, potential side effects, and adherence strategies
- Identify and resolve adverse drug events; participate in pharmacovigilance initiatives
- Support quality improvement, regulatory compliance, and interdepartmental projects
Qualification
- Doctor of Pharmacy (PharmD) from an ACPE-accredited program
- Active Massachusetts pharmacist license or eligibility to obtain licensure
- Minimum 2 years of hospital or health-system pharmacy experience
- Strong clinical knowledge across therapeutics and medication safety
- Excellent communication, collaboration, and leadership skills
- Proficiency with EMR/e-prescribing systems and MTM documentation
- Board Certification in Pharmacotherapy (BCPS) or related certification is a plus
