Responsibility
- Provide direct patient care through comprehensive medication reviews, MTM, and collaborative care teams.
- Conduct accurate medication reconciliation and resolve therapy issues across transitions of care.
- Deliver evidence-based recommendations to optimize pharmacotherapy and patient outcomes.
- Oversee immunization administration protocols and maintain vaccination records in the EHR.
- Support formulary management, cost containment, and utilization review.
- Educate patients and care teams on safe medication use, adverse effects, and adherence strategies.
- Mentor pharmacy students and interns; contribute to quality improvement initiatives.
Qualification
- PharmD degree; valid pharmacist license to practice in Massachusetts (or eligible for MA licensure).
- Minimum 2 years of clinical pharmacy experience in a hospital or health-system setting.
- Board-certified Pharmacotherapy Specialist (BCPS) or equivalent certification preferred.
- Strong knowledge of pharmacotherapy, pharmacokinetics, and drug interactions.
- Excellent communication, collaboration, and documentation skills; proficient in EHR systems (EPIC).
- Immunization certification and ability to administer vaccines; MA immunization training is required.
- Demonstrated commitment to patient safety, quality improvement, and continuous learning.
