Responsibility

  • Conduct comprehensive medication reviews and optimize drug therapy for inpatients, collaborating with physicians, nurses, and the care team.
  • Provide evidence-based pharmacotherapy guidance, therapeutic drug monitoring, and dosing adjustments as needed.
  • Participate in daily rounds and care planning to ensure safe and effective medication use.
  • Monitor for drug interactions, allergies, contraindications, renal/hepatic dosing, and duplicate therapies.
  • Lead drug utilization reviews, formulary management, and pharmacoeconomic assessments where applicable.
  • Promote medication safety initiatives, report adverse events, and contribute to pharmacovigilance activities.
  • Educate patients and families on medication regimens, side effects, and adherence strategies.
  • Mentor pharmacy technicians and interns, and contribute to continuous quality improvement projects.

Qualification

  • Doctor of Pharmacy PharmD from an ACPE-accredited program.
  • Active Massachusetts Pharmacist license or eligibility to obtain licensure.
  • Minimum 2 years of hospital or clinical pharmacy experience.
  • Strong knowledge of pharmacotherapy, drug interactions, and therapeutic monitoring.
  • Excellent written and verbal communication, collaboration, and teamwork skills.
  • Proficiency with Electronic Health Records and pharmacy software (Epic, CQMS) and reporting tools.
  • Board Certification in Pharmacotherapy (BCPS) or advanced credentials preferred.
  • Ability to thrive in a fast-paced, patient-centered hospital environment and demonstrate compassion under pressure.
