Join Bayview Health Systems' premier Hospital Pharmacy team in San Francisco. This is a patient-facing Clinical Pharmacist role dedicated to optimizing medication therapy, ensuring safety, and collaborating with multidisciplinary teams.
You will deliver high-quality pharmaceutical care across acute care units, support formulary decisions, and participate in pharmacovigilance initiatives while upholding the highest standards of patient safety.
Responsibility
- Provide comprehensive medication therapy management (MTM) and clinical drug consultations for inpatients.
- Collaborate with physicians, nurses, and case managers during daily rounds to optimize therapy.
- Verify and dispense medications accurately, ensuring proper labeling and dosing accuracy.
- Conduct drug utilization reviews, monitor adverse events, and report safety concerns per policy.
- Assist in formulary management, guideline development, and evidence-based protocol optimization.
- Participate in sterile compounding and preparation per USP standards (if applicable).
- Educate patients and caregivers on medication regimens, potential interactions, and adherence strategies.
- Document clinical interventions in the electronic health record and track outcomes.
Qualification
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school.
- Active Pharmacist License in California (or eligible to obtain).
- Minimum 2 years of hospital or health-system pharmacy experience preferred.
- Board Certification in Pharmacotherapy (BCPS) or commitment to pursue within 2 years is a plus.
- Strong knowledge of pharmacokinetics, drug interactions, and evidence-based medicine.
- Excellent communication, teamwork, and patient-centered care skills.
- Proficient with electronic health records (EPIC, Cerner) and pharmacy information systems.
