Responsibility
- Provide direct patient care through comprehensive medication reviews and individualized counseling to optimize therapy and outcomes.
- Review, verify, and process prescriptions for accuracy, safety, and appropriate therapy.
- Collaborate with prescribers and healthcare teams to optimize pharmacotherapy plans and resolve drug-related problems.
- Lead medication therapy management (MTM) initiatives and chronic disease management programs.
- Oversee medication dispensing, inventory management, and regulatory compliance within the pharmacy operations.
- Ensure accurate documentation, HIPAA compliance, and data integrity in electronic health records.
- Educate and mentor pharmacists, interns, and pharmacy technicians to uphold clinical and service standards.
- Contribute to quality improvement, safety initiatives, and patient education campaigns.
Qualification
- PharmD degree from an accredited school; valid New York Pharmacist license (or eligibility).
- Minimum 2 years of clinical pharmacy experience in a retail, hospital, or integrated health setting.
- Strong knowledge of pharmacotherapy, medication safety, and evidence-based care.
- Proven skills in patient counseling, MTM, and chronic disease management.
- Experience with electronic health records and pharmacy software systems (e.g., Epic, Cerner, PrimeRx).
- Excellent communication, collaboration, and problem-solving abilities.
