HopeBridge Social Services is seeking a licensed Clinical Social Worker to join our community-focused team in Chicago, IL. This full-time role provides direct client services, crisis intervention, and collaboration with multidisciplinary teams to promote safety, resilience, and well-being for individuals and families.

Responsibility
- Conduct psychosocial assessments and develop individualized treatment plans in collaboration with clients and families.
- Provide evidence-based individual, family, and group counseling as appropriate to client needs.
- Deliver crisis intervention, safety planning, and risk assessment to prevent harm.
- Coordinate with healthcare providers, schools, housing agencies, and community organizations to ensure holistic support.
- Maintain accurate, timely documentation and case notes in the electronic health record (EHR) system.
- Advocate for clients to access benefits and resources, and promote self-determination and empowerment.
- Participate in program development, quality improvement, supervision, and cross-team collaboration to enhance service delivery.
Qualification
- Master's degree in Social Work (MSW) from an accredited program.
- Current Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois or eligibility for licensure.
- Minimum of 2-3 years of experience in healthcare, behavioral health, or community-based social services.
- Strong knowledge of community resources, benefits systems, and enrollment processes.
- Excellent communication, assessment, and crisis intervention skills; ability to work with diverse populations.
- Demonstrated cultural competence and trauma-informed practice; commitment to anti-oppressive values.
- Proficiency with electronic health records (EHR), documentation standards, and data privacy.
