Responsibility
- Provide comprehensive primary care and general medicine services to adult patients across the lifespan.
- Perform thorough assessments, diagnose conditions, order appropriate tests, and develop evidence-based treatment plans.
- Manage chronic diseases (hypertension, diabetes, hyperlipidemia) and coordinate care with specialists as needed.
- Document all encounters accurately in the EHR and monitor quality metrics, patient safety, and outcomes.
- Collaborate with nurses, care coordinators, and behavioral health professionals to deliver holistic care.
- Participate in on-site and telemedicine clinics, including occasional after-hours coverage as part of a rotation.
- Promote preventive care, vaccinations, screenings, and patient education to improve population health.
Qualification
- Medical Doctor (MD) or Doctor of Osteopathy (DO) licensed to practice in Illinois; Board-certified/Board-eligible in Internal Medicine or Family Medicine.
- Minimum 2 years of relevant clinical experience in primary care preferred.
- Strong communication, empathy, and patient rapport-building skills.
- Proficient in EHR systems (Epic or comparable platform) and clinical decision support tools.
- Ability to work effectively within a multidisciplinary team and diverse patient population.
- Current CPR/BLS certification and valid medical malpractice insurance coverage.
