Responsibility
- Provide comprehensive medical care for adult patients with emphasis on prevention, diagnosis, and management of acute and chronic illnesses.
- Develop and execute individualized care plans in collaboration with patients, families, and a multidisciplinary team.
- Coordinate care across services including diagnostics, specialty referrals, and transitions of care.
- Perform routine evaluations, preventive screenings, and evidence based treatment adjustments.
- Manage complex cases with multimorbidity, including hospital admissions and post discharge planning when needed.
- Educate patients on health maintenance, treatment options, and shared decision making to improve outcomes.
- Contribute to quality improvement initiatives and participate in clinical research activities.
- Mentor and support medical students and junior clinicians in a collaborative learning environment.
Qualification
- Medical degree from an accredited medical school and a valid license to practice in Massachusetts
- Board certified or board eligible in Internal Medicine
- Completed residency in Internal Medicine or a closely related program
- Minimum of 3 years clinical experience in adult medicine
- Proven commitment to patient centered care and continuous quality improvement
- Excellent communication and teamwork skills with diverse patient populations
- Proficiency with electronic medical records and data driven care coordination
- Eligibility for academic appointment or teaching roles if desired
