UrbanCare Medical Group is seeking a compassionate General Medicine Physician to join our outpatient practice in San Francisco. You will provide comprehensive primary care across the lifespan, manage chronic diseases, and collaborate with a multidisciplinary team to deliver exceptional patient outcomes. Responsibility
- Provide comprehensive primary care across the lifespan, including preventive services and wellness counseling
- Diagnose and manage acute and chronic illnesses with evidence-based treatment plans
- Develop and monitor individualized care plans, including chronic disease management
- Collaborate with specialists, nurses, and care coordinators to optimize patient outcomes
- Document patient encounters accurately in the EHR and ensure data quality
- Participate in quality improvement initiatives, peer review, and continuing medical education
- Support after-hours clinics or on-call coverage as needed
Qualification
- MD/DO from an accredited medical school; board-certified or board-eligible in Family Medicine or Internal Medicine
- Active California medical license or license-eligible with ability to obtain license
- Minimum of 3 years of outpatient primary care experience
- Strong diagnostic, clinical reasoning, and communication skills
- Proficiency with EHR systems (Epic preferred) and documentation standards
- Ability to work collaboratively in a diverse, multidisciplinary team
- Current BLS certification; ACLS preferred
