CityCare Health Partners is seeking a compassionate and experienced General Medicine Physician to join our trusted primary care team in New York City. This full-time role offers a collaborative environment, modern facilities, and opportunities for clinical leadership.
As a General Medicine Physician, you will provide comprehensive, evidence-based care across adult medical conditions, emphasizing preventive care and patient education. You will work with a multidisciplinary team to deliver outstanding patient outcomes.
Responsibility
- Deliver comprehensive primary care to adult patients, including new patient evaluations and follow-up visits.
- Develop and implement individualized diagnostic and treatment plans based on evidence-based guidelines.
- Manage acute and chronic illnesses, including chronic disease management and preventive care.
- Coordinate care with specialists and ancillary services, ensuring seamless patient transitions.
- Document medical findings and treatment plans accurately in the electronic health record (EHR).
- Educate patients on lifestyle modifications, medications, and adherence to therapy.
- Participate in quality improvement initiatives, case conferences, and continuing medical education.
- Support mentorship of medical trainees and collaborate with a diverse clinical team.
Qualification
- MD or DO degree from an accredited medical school.
- Board certification or eligibility in Internal Medicine (ABIM or equivalent).
- Active, unrestricted medical license to practice in New York or eligibility for licensure in New York.
- Minimum of 3 years of clinical experience in general internal medicine or primary care (preferred).
- Strong proficiency with electronic health records (EHR) and clinical decision support tools.
- Excellent communication and interpersonal skills; ability to work collaboratively with a diverse patient population.
- Ability to manage a broad scope of medical conditions with a patient-centered approach; commitment to preventive care.
- ACLS/BLS certification (or willingness to obtain).
