Job description

CityCare Health Partners is seeking a compassionate and experienced General Medicine Physician to join our trusted primary care team in New York City. This full-time role offers a collaborative environment, modern facilities, and opportunities for clinical leadership.

As a General Medicine Physician, you will provide comprehensive, evidence-based care across adult medical conditions, emphasizing preventive care and patient education. You will work with a multidisciplinary team to deliver outstanding patient outcomes.

Responsibility

  • Deliver comprehensive primary care to adult patients, including new patient evaluations and follow-up visits.
  • Develop and implement individualized diagnostic and treatment plans based on evidence-based guidelines.
  • Manage acute and chronic illnesses, including chronic disease management and preventive care.
  • Coordinate care with specialists and ancillary services, ensuring seamless patient transitions.
  • Document medical findings and treatment plans accurately in the electronic health record (EHR).
  • Educate patients on lifestyle modifications, medications, and adherence to therapy.
  • Participate in quality improvement initiatives, case conferences, and continuing medical education.
  • Support mentorship of medical trainees and collaborate with a diverse clinical team.

Qualification

  • MD or DO degree from an accredited medical school.
  • Board certification or eligibility in Internal Medicine (ABIM or equivalent).
  • Active, unrestricted medical license to practice in New York or eligibility for licensure in New York.
  • Minimum of 3 years of clinical experience in general internal medicine or primary care (preferred).
  • Strong proficiency with electronic health records (EHR) and clinical decision support tools.
  • Excellent communication and interpersonal skills; ability to work collaboratively with a diverse patient population.
  • Ability to manage a broad scope of medical conditions with a patient-centered approach; commitment to preventive care.
