Responsibility
- Provide comprehensive primary care to adult patients in outpatient and hospital settings, focusing on preventive medicine and chronic disease management.
- Develop individualized treatment plans, monitor progress, and adjust therapies based on evidence-based guidelines.
- Collaborate with specialists, nurses, and care coordinators to optimize patient outcomes and care transitions.
- Lead patient education efforts, including risk factor modification and healthy lifestyle counseling.
- Participate in quality improvement initiatives, clinical audits, and performance metrics.
- Document patient encounters accurately in the electronic health record and ensure data integrity.
- Respond to acute illness and triage urgent cases in a timely and safe manner.
Qualification
- Medical Doctor (MD) or DO licensed to practice in New York or eligible for licensure in NY.
- Board-certified or board-eligible in internal medicine or general medicine.
- Minimum 3 years of clinical experience in adult primary care or internal medicine.
- Strong diagnostic and clinical decision-making skills with a patient-centered approach.
- Excellent communication, collaboration, and ability to work within a multidisciplinary team.
- Proficiency with electronic health records and basic data analytics.
- Commitment to ongoing professional development and quality improvement.
- Authorization to work in the United States; eligibility for NY medical licensure.
