Responsibility

  • Provide comprehensive primary care and longitudinal management for adult patients across diverse conditions.
  • Perform accurate assessment, differential diagnosis, and evidence-based treatment planning.
  • Coordinate care with specialists, nurses, pharmacists, and allied health professionals.
  • Manage chronic conditions such as diabetes, hypertension, dyslipidemia, and cardiovascular risk with guideline-concordant care.
  • Deliver patient education, preventive care, and lifestyle modification counseling.
  • Document clinical encounters accurately in the EMR and ensure data integrity.
  • Participate in quality improvement projects, audits, and clinic-based research initiatives.
  • Mentor medical students and residents; contribute to continuing medical education efforts.

Qualification

  • Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) with eligibility to practice in Ontario.
  • Board-certified or eligible in Family Medicine (CCFP or equivalent).
  • Licensed to practice in Ontario and in good standing with the College of Physicians and Surgeons of Ontario.
  • Minimum 2 years of post-residency clinical experience in General Medicine or Family Medicine (preferred).
  • Excellent communication, empathy, and patient-centered care approach.
  • Proficiency with electronic medical records (EMR) and digital health tools.
  • Ability to work collaboratively in a multidisciplinary team and participate in quality improvement.
  • Current CPR/BLS certification; ACLS preferred.
