Responsibility
- Conduct comprehensive psychosocial assessments and develop individualized care plans.
- Provide therapeutic counseling to clients, including individuals, families, and groups.
- Advocate for client access to resources, community services, and benefits.
- Maintain accurate, up-to-date clinical documentation and case records.
- Collaborate with interdisciplinary teams to coordinate and optimize client care.
- Facilitate crisis intervention and support during emergencies.
- Participate in case conferences and perform ongoing client progress evaluations.
- Stay current with best practices, attending professional development and training sessions as required.
Qualification
- Master's degree in Social Work (MSW) from an accredited institution.
- Active LCSW license in the state of California.
- Minimum 2 years of clinical experience working with diverse client populations.
- Strong understanding of trauma-informed and client-centered care.
- Exceptional communication, advocacy, and case management skills.
- Proficiency in electronic health records (EHR) and documentation standards.
- Commitment to ethical practice and cultural competence.
- Bilingual abilities (Spanish/English) highly desirable.
