Responsibility
- Provide clinical counseling and support to individuals, families, and groups
- Conduct psychosocial assessments and develop individualized care plans
- Collaborate with multidisciplinary teams to coordinate holistic services
- Maintain accurate case notes and documentation in compliance with agency and regulatory standards
- Facilitate referrals to community resources and manage client follow-up
- Deliver crisis intervention and advocacy as needed
- Engage in ongoing training and professional development activities
- Uphold the organization's mission, values, and ethical standards
Qualification
- Master’s Degree in Social Work from an accredited institution
- Active LCSW license in the state of Illinois
- Minimum 2 years' experience in clinical social work or related field
- Demonstrated expertise in trauma-informed and culturally competent care
- Superb interpersonal and communication skills
- Proficiency in electronic health records and documentation standards
- Capacity to manage complex caseloads in fast-paced environments
- Bilingual (English/Spanish) a strong advantage
