Responsibility
- Provide individual, group, and family counseling for clients from diverse backgrounds.
- Coordinate care plans with medical professionals and community agencies.
- Conduct psychosocial assessments, crisis intervention, and ongoing case management.
- Document all client interactions accurately in the electronic health record system.
- Advocate for client access to essential services, including housing, mental health, and employment support.
- Facilitate workshops and support groups on mental health awareness and resources.
- Participate in interdisciplinary team meetings to ensure holistic care delivery.
- Maintain up-to-date professional licensure and participate in continuing education.
Qualification
- Master's degree in Social Work (MSW) from an accredited institution.
- Active LCSW or eligibility to obtain Illinois LCSW within 3 months of hire.
- Minimum 2 years’ experience in clinical or community-based social work.
- Strong knowledge of trauma-informed care and community resources.
- Exceptional verbal, written, and interpersonal communication skills.
- Proficient in EMR and case documentation.
- Bilingual (Spanish/English) highly preferred.
- Demonstrated commitment to cultural competence and equity in service delivery.
