BrightPath Community Services is seeking a compassionate and experienced Senior Social Worker to join our Chicago team. This role provides direct client support, case management, and community collaboration to improve outcomes for families and individuals in need. You will work within a multidisciplinary team to assess needs, develop individualized care plans, and connect clients with essential services.

Responsibility
- Conduct comprehensive biopsychosocial assessments to identify client needs and risks.
- Develop and monitor evidence-based, person-centered care plans in collaboration with clients and families.
- Coordinate services across housing, employment, health, and legal supports to promote stability and well-being.
- Advocate for clients within schools, courts, and social systems to ensure access to resources.
- Provide crisis intervention, safety planning, and counseling as needed.
- Document services accurately in electronic case management systems and maintain privacy compliance.
- Engage in community outreach to connect with at-risk populations and build trust with local organizations.
Qualification
- Master's degree in Social Work or related field from an accredited program.
- Licensed Clinical Social Worker (LCSW) or equivalent licensure preferred; eligibility required.
- Minimum 3 years of direct social work experience, preferably in child welfare, family services, or community mental health.
- Strong assessment, case management, and crisis intervention skills.
- Excellent communication, collaboration, and cultural humility in diverse communities.
- Proficiency with EMR systems and client privacy regulations (HIPAA).
- Ability to work a flexible schedule, including occasional evenings or weekends for client appointments.
