BrightPath Community Services is seeking a compassionate and results driven Social Worker to join our interdisciplinary team in Chicago, IL. This full-time role focuses on delivering client-centered support to vulnerable families and individuals, assisting with safety planning, access to services, and long-term outcomes.

Responsibility
- Conduct comprehensive psychosocial assessments and risk evaluations
- Develop and implement individualized service plans in collaboration with clients
- Coordinate resources across housing, education, healthcare, and legal services
- Provide crisis intervention and de-escalation as needed
- Advocate for client needs with schools, courts, and community agencies
- Maintain accurate documentation in electronic case management systems and reporting
- Monitor progress, adjust plans, and provide ongoing counseling and referrals
- Participate in multidisciplinary team meetings and quality improvement initiatives
Qualification
- Master's in Social Work (MSW) from an accredited program; LCSW/LSW licensure in Illinois preferred
- Minimum 2 years of experience in social services, child welfare, or mental health
- Knowledge of trauma informed care and evidence based practices
- Strong assessment, case planning, and advocacy skills
- Excellent written and verbal communication; ability to build rapport with diverse populations
- Proficiency with electronic case management systems and data entry
- Valid driver's license and reliable transportation or ability to travel within the service area
- Bilingual in English and Spanish is a plus
