Job description

Responsibility

  • Conduct comprehensive psychosocial assessments and risk evaluations
  • Develop and implement individualized service plans in collaboration with clients
  • Coordinate resources across housing, education, healthcare, and legal services
  • Provide crisis intervention and de-escalation as needed
  • Advocate for client needs with schools, courts, and community agencies
  • Maintain accurate documentation in electronic case management systems and reporting

Qualification

  • Strong assessment, case planning, and advocacy skills
  • Excellent written and verbal communication; ability to build rapport with diverse populations
  • Proficiency with electronic case management systems and data entry
  • Valid driver's license and reliable transportation or ability to travel within the service area
  • Bilingual in English and Spanish is a plus
