HopePath Community Services is seeking a compassionate Licensed Social Worker (LCSW) to join our multidisciplinary team in San Francisco. This role focuses on delivering client-centered care to individuals and families facing housing instability, mental health challenges, and other social barriers. You will assess needs, develop treatment plans, coordinate resources, and advocate for clients within the community. We value equity, confidentiality, and trauma-informed practice. Responsibility
- Provide individual and family counseling, crisis intervention, and case management to clients affected by housing instability, mental health challenges, or violence.
- Develop and implement person-centered treatment plans, progress notes, and discharge plans in collaboration with clients and partners.
- Assess needs, safety concerns, and risk factors; refer to community resources and services as appropriate.
- Advocate for clients in navigating healthcare, housing, education, and legal systems; coordinate with hospitals, social services, and community organizations.
- Conduct outreach to underserved communities; participate in outreach events, workshops, and trainings.
- Document ethically and maintain confidential records in compliance with HIPAA and agency policies.
- Collaborate with a multidisciplinary team to monitor outcomes and improve service delivery using data-driven approaches.
- Provide supervision or mentorship to junior staff or interns when applicable.
Qualification
- Master of Social Work (MSW) from an accredited program; valid LCSW licensure in California or eligibility for licensure transfer.
- Minimum 2-3 years of direct social work experience in clinical or community settings; trauma-informed care experience preferred.
- Strong clinical assessment, crisis intervention, and safety planning skills.
- Excellent communication, empathy, and cultural humility; ability to work with diverse populations.
- Knowledge of community resources in the San Francisco Bay Area; proficiency in documentation and case management software.
- Ability to work independently and as part of a collaborative team; flexible schedule including some evenings or weekends if required.
- Commitment to ethics, confidentiality, and patient rights; ability to maintain HIPAA compliance.
- Bilingual in English and Spanish or other languages commonly spoken in the service area is a plus.
