NovaCare Community Services is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic care team in New York City. In this role, you will provide evidence-based counseling, crisis intervention, and comprehensive case management to individuals, families, and at-risk youth within our community-based programs. You will collaborate with an interdisciplinary team to create sustainable care plans that promote resilience, safety, and well-being.

Responsibility
- Conduct comprehensive psychosocial assessments and develop person-centered treatment plans.
- Provide individual, family, and group counseling using evidence-based modalities.
- Deliver crisis intervention and safety planning for high-risk clients.
- Coordinate with schools, healthcare providers, and community agencies to arrange services.
- Advocate for clients to access housing, benefits, and social supports.
- Maintain accurate documentation in EMR systems and ensure compliance with regulatory standards.
- Participate in case conferences and multidisciplinary team meetings to monitor progress and adjust plans as needed.
Qualification
- Master's degree in Social Work (MSW) from an accredited program; LCSW licensure in New York or eligibility for licensure within 6 months.
- Minimum 2 years of post-graduate social work experience in clinical or community-based settings.
- Strong counseling skills with proficiency in evidence-based modalities (e.g., CBT, DBT, trauma-focused care).
- Excellent assessment, documentation, and organizational skills; ability to manage complex cases.
- Knowledge of community resources, benefits programs, and housing supports; familiarity with NYC service networks.
- Commitment to ethical practice, client confidentiality, and anti-discrimination principles; ability to work collaboratively in a diverse environment.
