Responsibility
- Conduct comprehensive psychosocial assessments and risk evaluations for adults, families, and youth.
- Provide evidence-based individual, family, and group counseling using appropriate modalities.
- Coordinate care across multidisciplinary teams to ensure seamless service delivery and resource connection.
- Develop, implement, and monitor personalized care plans, measuring progress and adjusting as needed.
- Deliver crisis intervention, safety planning, and rapid linkage to community resources.
- Advocate for clients navigating housing, employment, healthcare, and social services.
- Document all interactions in the EMR with accuracy and HIPAA-compliant confidentiality.
- Engage in supervision, training, and quality improvement initiatives to enhance practice.
Qualification
- MSW from an accredited program and current Oregon LCSW licensure (or eligibility and plan to obtain).
- 2+ years of clinical social work experience in behavioral health, child welfare, or community-based settings.
- Strong assessment, counseling, crisis intervention, and case management skills.
- Trauma-informed, culturally competent practice with a commitment to equity and justice.
- Excellent written/verbal communication and proficient use of EMR and HIPAA compliance.
- Ability to work collaboratively in a diverse, fast-paced environment and manage competing priorities.
- Knowledge of local community resources, housing, and healthcare systems.
