We are seeking a compassionate Licensed Clinical Social Worker LCSW to provide clinical services, advocacy, and care coordination. As an LCSW on our team, you will work directly with clients to assess needs, develop person-centered plans, and connect them to essential resources. You will also contribute to program development, community outreach, and quality improvement initiatives that advance our commitment to equity and wellness.
Responsibility
- Conduct comprehensive psychosocial assessments and develop individualized treatment plans in collaboration with clients and families.
- Provide evidence-based counseling and crisis intervention to individuals, families, and groups.
- Coordinate services and link clients to housing, financial assistance, healthcare, and community supports.
- Advocate for clients across systems, document outcomes, and ensure timely follow-up.
- Maintain accurate EMR documentation, ensure compliance with policies, and participate in quality assurance processes.
- Collaborate with multidisciplinary teams to monitor progress and modify plans as needed.
- Engage in community outreach and education to reduce barriers to care and increase access to services.
Qualification
- Master of Social Work (MSW) from an accredited program; licensure as LCSW in Illinois or eligibility to obtain licensure within 6 months.
- Minimum of 2-3 years of clinical social work experience, preferably in community-based or nonprofit settings.
- Strong knowledge of trauma-informed care, motivational interviewing, and evidence-based practices.
- Excellent communication, active listening, and client-centered interviewing skills.
- Ability to assess risk, develop safety plans, and manage crisis situations.
- Proficient in EMR systems, data entry, and program reporting; detail-oriented and organized.
- Commitment to cultural humility, equity, and serving diverse communities; valid driver's license and reliable transportation.
