Responsibility
- Provide patient-centered pharmaceutical care by reviewing prescriptions, counseling patients, and addressing questions about medications and proper usage.
- Verify and dispense prescriptions accurately while ensuring appropriate dosing, interactions checks, and allergy considerations.
- Conduct medication therapy management to optimize efficacy, safety, and adherence for each patient.
- Collaborate with physicians, specialists, and the broader care team to reconcile therapies and improve outcomes.
- Oversee inventory management, controlled substance compliance, and pharmacy operational standards.
- Maintain accurate patient records in the EMR system and document clinical interventions.
- Participate in immunization programs, staff training, and quality assurance initiatives.
Qualification
- Active Pharmacist license in Illinois with a PharmD degree from an accredited program.
- Minimum of 2 years of community or clinical pharmacy experience preferred.
- Strong patient counseling and communication skills with a patient-first approach.
- Proficiency with pharmacy software, EMR/EHR systems, and medication therapy management tools.
- In-depth knowledge of federal and state pharmacy regulations, including dispensing, documentation, and safety standards.
- Ability to work in a fast-paced environment, manage priorities, and collaborate with a multidisciplinary team.
- Immunization certification or eligibility to become certified within the state.
- Attention to detail, critical thinking, and a commitment to continuous learning.
