HopeBridge Community Services is a mission-driven nonprofit dedicated to empowering individuals and families to achieve stability and well-being. We are seeking a Licensed Social Worker to provide compassionate, person-centered support in our outpatient clinic in New York City. This full-time role collaborates with a multidisciplinary team to assess needs, develop service plans, advocate for resources, and ensure culturally competent care.

Responsibility
- Conduct comprehensive psychosocial assessments to determine strengths, needs, and risks, and document findings in the electronic health record.
- Develop and implement individualized service plans in partnership with clients and families, aligning goals with community resources.
- Coordinate case management across healthcare, housing, education, and social services to remove barriers to stability.
- Provide crisis intervention, safety planning, and de-escalation as needed to protect client safety and well-being.
- Advocate for clients with external agencies, schools, and employers to secure essential services and supports.
- Collaborate with physicians, therapists, and other professionals to ensure holistic, integrated care.
- Maintain accurate, confidential documentation and comply with all regulatory and agency policies.
- Engage in community outreach and client education to raise awareness of available resources and programs.
Qualification
- Master of Social Work (MSW) from an accredited program.
- Licensed Social Worker (LCSW) or LMSW in New York, or eligibility to obtain licensure within 6 months.
- Minimum of 2 years' experience in case management, clinical or community-based social work.
- Strong knowledge of trauma-informed care, cultural humility, and ethical practice.
- Excellent written and verbal communication skills with the ability to build rapport with diverse populations.
- Proficiency in electronic medical records and social service software; attention to detail and strong organizational skills.
- Bilingual abilities (English plus Spanish or other language) preferred but not required.
