Responsibility
- Develop and manage individualized service plans for clients based on assessments and goals.
- Coordinate referrals to housing, employment, healthcare, financial assistance, and other supports.
- Conduct intake assessments, safety planning, and crisis intervention as needed.
- Monitor client progress, document outcomes, and maintain accurate records in our case management system.
- Collaborate with multidisciplinary teams and community partners to ensure holistic care.
- Advocate for client rights and ensure culturally responsive, trauma-informed service delivery.
- Provide coaching and supported referrals to promote client independence and self-sufficiency.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field; Master's degree preferred.
- Minimum of 2 years of case management or direct social services experience.
- Strong knowledge of local community resources and referral networks.
- Excellent written and verbal communication skills; crisis intervention and de-escalation abilities.
- Experience with trauma-informed care and commitment to equity and inclusion.
- Proficiency with case management software, data entry, and basic reporting.
- Bilingual in English and Spanish (or other languages) is a plus.
