Responsibility

  • Develop and manage individualized service plans for clients based on assessments and goals.
  • Coordinate referrals to housing, employment, healthcare, financial assistance, and other supports.

Qualification

  • Bachelor's degree in Social Work, Psychology, Sociology, or a related field; Master's degree preferred.
  • Minimum of 2 years of case management or direct social services experience.
  • Strong knowledge of local community resources and referral networks.
  • Excellent written and verbal communication skills; crisis intervention and de-escalation abilities.
  • Experience with trauma-informed care and commitment to equity and inclusion.
  • Proficiency with case management software, data entry, and basic reporting.
  • Bilingual in English and Spanish (or other languages) is a plus.
