BrightPath Community Services is seeking a dedicated Social Services Case Manager to join our team in Toronto. Responsibility
- Conduct comprehensive client assessments to identify needs, risks, and strengths.
- Develop and implement individualized service plans in collaboration with clients and families.
- Coordinate referrals to housing, employment, financial assistance, mental health, and other community resources.
- Monitor progress, document outcomes, and adjust plans to maximize impact.
- Advocate for clients to ensure access to entitlements, benefits, and rights.
- Maintain accurate case notes and data in confidential client management systems.
- Collaborate with multidisciplinary teams, social workers, and community organizations to optimize supports.
Qualification
- Bachelor's degree in Social Work, Counseling, Psychology, or related field; Master's preferred.
- Eligible for or registered with the provincial social work regulatory body or equivalent.
- Minimum 2 years of experience in case management or direct client services in social services.
- Strong communication, empathy, and crisis intervention skills.
- Knowledge of housing, employment, welfare programs, and community resources in the Toronto area.
- Proficiency with case management software and Microsoft Office; commitment to data privacy.
- Bilingual English/French (preferred) or commitment to serve diverse communities.
- Ability to work evenings or weekends as required; valid driver's license may be preferred in some roles.
