Responsibility
- Conduct comprehensive psychosocial assessments and risk screens for clients across age groups.
- Develop, implement, and monitor individualized service plans aligned with client goals.
- Provide counseling, crisis intervention, and referral to community-based supports.
- Coordinate care across healthcare, housing, education, and social services partners.
- Document case notes, maintain compliance with regulations, and ensure data accuracy in electronic records.
- Advocate for client needs and assist with benefits applications and resource navigation.
- Facilitate group sessions and support networks to promote resilience and social integration.
- Engage in continuous quality improvement and participate in supervision and team meetings.
Qualification
- Master of Social Work (MSW) from an accredited program
- Current LMSW or LCSW license in good standing (or eligible to obtain in Oregon)
- Minimum of 2 years of direct clinical social work experience
- Experience with trauma-informed care, crisis intervention, and client advocacy
- Strong assessment, documentation, and organizational skills
- Excellent communication and collaboration with multidisciplinary teams
- Commitment to equity, cultural humility, and ethical practice
- Proficiency with electronic health records and community resource databases
