HopeBridge Family Services is seeking a dedicated Social Services Case Manager to support families and individuals in Chicago. You will assess client needs, develop tailored service plans, and connect clients with essential community resources. As part of a multidisciplinary team, you will advocate for clients, track progress, ensure compliance with policy, and maintain confidential records with precision and empathy.
Responsibility
- Conduct client assessments and intake to determine needs and goals.
- Develop and implement individualized service plans in collaboration with clients and families.
- Coordinate housing, healthcare, employment, education, and social service resources.
- Advocate for clients with agencies, landlords, and employers to remove barriers.
- Maintain accurate, confidential case notes and documentation; track outcomes and progress.
- Conduct outreach to at-risk populations and participate in community partnerships.
- Monitor compliance with service plans and adjust as needed; provide crisis support and referrals.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or related field.
- LSW or LMSW licensure preferred; active/unrestricted license.
- 2+ years of experience in social services or case management.
- Excellent communication, empathy, and cultural sensitivity.
- Ability to manage caseload, prioritize, and maintain organization.
- Knowledge of local community resources and government assistance programs.
- Proficiency with case management software and data entry; MS Office.
