Responsibility
- Conduct comprehensive client assessments to identify social, emotional, and economic needs.
- Develop and implement individualized service plans for clients and their families.
- Provide counseling, crisis intervention, and advocacy for clients.
- Facilitate access to community resources, housing, healthcare, and support services.
- Maintain detailed case documentation and comply with confidentiality requirements.
- Collaborate closely with healthcare providers, educators, and community organizations.
- Assess risk factors and develop safety plans when necessary.
- Lead support groups and educational workshops to address client needs.
Qualification
- Master’s degree in Social Work (MSW) from an accredited program.
- Active Illinois state licensure (LSW or LCSW) required.
- Minimum 2 years of experience in social services or clinical casework.
- Strong interpersonal, organizational, and communication skills.
- Proficient in crisis intervention and resource coordination.
- Ability to manage diverse caseloads in a fast-paced environment.
- Knowledge of local community resources in Chicago is preferred.
- Bilingual (Spanish/English) skills are an asset.
