Harborview Family Services is seeking a compassionate Licensed Social Worker to join our dynamic care team in Boston. In this role you will partner with clients, families, and community partners to develop integrated support plans, connect individuals to housing, benefits, and clinical services, and advocate for access to essential resources. You will contribute to a collaborative team that values cultural humility, ethical practice, and client-centered care.
Responsibility
- Conduct comprehensive biopsychosocial assessments and risk evaluations
- Develop and monitor individualized service plans in collaboration with clients and families
- Coordinate care across multi-disciplinary teams and community partners
- Provide crisis intervention, de-escalation, and safety planning as needed
- Advocate for clients to access housing, benefits, mental health services, and supportive programs
- Maintain accurate, timely documentation and comply with confidentiality and regulatory requirements
- Facilitate referrals, resource navigation, and follow-up to ensure service continuity
- Engage in reflective supervision and participate in ongoing professional development
Qualification
- Master of Social Work (MSW) from an accredited program; current licensure or eligibility for licensure (LSW/LICSW/LCSW) in good standing
- Minimum 2 years of experience in social work, case management, or related settings (public health, child welfare, or behavioral health)
- Strong knowledge of community resources, benefits programs, and housing options
- Excellent written and verbal communication, with strong interviewing and assessment skills
- Trauma-informed, culturally competent practice and commitment to equity
- Proficiency with electronic case management systems and documentation standards
- Ability to manage competing priorities, meet deadlines, and work collaboratively in a team
- Willingness to work occasional evenings or weekends as needed and a valid driver's license may be required
