Job description

Responsibility

  • Lead comprehensive case management for individuals and families, coordinating with healthcare, housing, education, and social services to create integrated plans.
  • Provide crisis intervention, risk assessment, and safety planning to ensure client safety and well-being.
  • Advise and advocate for clients navigating community resources, including housing, benefits, and child welfare services.
