BrightPath Community Services is seeking a dedicated Senior Social Worker to empower individuals and families through holistic support, advocacy, and evidence-based interventions. As a key member of our multidisciplinary team, you will lead case management, crisis response, and resource navigation, while shaping program development and community partnerships.

Responsibility
- Lead comprehensive case management for individuals and families, coordinating with healthcare, housing, education, and social services to create integrated plans.
- Provide crisis intervention, risk assessment, and safety planning to ensure client safety and well-being.
- Advise and advocate for clients navigating community resources, including housing, benefits, and child welfare services.
- Coordinate interdisciplinary team meetings and develop client-centered care plans with measurable goals and outcomes.
- Build and maintain strong relationships with community partners, schools, healthcare providers, and public agencies.
- Document case progress, maintain compliant records, and prepare timely reports for funders and regulatory bodies.
- Contribute to program development, evaluation, and staff training to enhance service delivery and cultural responsiveness.
Qualification
- Master of Social Work (MSW) or equivalent; LCSW licensure preferred or eligible for licensure in California.
- Minimum of 5 years of clinical social work experience with diverse populations.
- Strong foundation in trauma-informed care and client-centered practice.
- Excellent assessment, crisis intervention, and safety planning skills.
- Cultural humility and ability to engage with clients from varied backgrounds.
- Excellent written and verbal communication, documentation, and organizational skills; proficiency with case management software and HIPAA compliance.
- Ability to manage a high-volume caseload in a fast-paced environment while maintaining attention to detail and deadlines.
- Collaborative mindset with demonstrated ability to build partnerships with community organizations and public agencies.
