Harborview Community Services is seeking a compassionate Senior Social Services Case Manager to join our mission-driven team in Portland, Oregon. Responsibility
- Perform thorough intake assessments, risk evaluations, and eligibility determinations for services.
- Develop and monitor individualized service plans in collaboration with clients and families.
- Coordinate with housing programs, health care providers, education services, and community resources to secure essential supports.
- Advocate on behalf of clients to remove barriers and ensure access to benefits and entitlements.
- Provide crisis intervention, de-escalation, and safety planning as needed.
- Maintain accurate, compliant documentation and timely case notes in the agency's EMR system.
- Engage in regular case reviews, supervision, and quality improvement initiatives to enhance outcomes.
Qualification
- Bachelor's degree in Social Work, Sociology, Psychology, or a related field; Master's preferred.
- Active and valid state licensure or credential such as LMSW or LCSW preferred.
- Minimum 2 years of experience in case management or related social services.
- Strong knowledge of community resources, housing programs, health services, and eligibility requirements.
- Excellent written and verbal communication, advocacy, and cultural competency skills.
- Ability to manage sensitive information with confidentiality and adhere to ethical guidelines.
- Proficiency with case management software and EMR systems; strong organizational skills and attention to detail.
- Valid driver's license and reliable transportation for community based work if required.
