BrightPath Community Services is seeking a compassionate and results-driven Social Services Case Manager to join our Chicago team. This full-time role focuses on client-centered support, assessment, and advocacy to help individuals and families access essential services.
In this role, you will collaborate with multi-disciplinary teams, develop individualized service plans, and connect clients to housing, healthcare, education, employment, and social supports. You will also document case progress, monitor outcomes, and ensure compliance with program guidelines.
Responsibility
- Respond to client intake and assess immediate needs
- Develop and implement individualized case plans
- Coordinate with housing, healthcare, education, and social services
- Advocate for clients and monitor progress toward goals
- Maintain accurate documentation and data entry in the case management system
- Provide crisis intervention and referral services as needed
- Collaborate with community partners to expand access to resources
- Prepare reports and participate in case conferences
Qualification
- Bachelor's degree in Social Work, Psychology, or a related field; Master's preferred
- Current or willingness to obtain licensure/credentials (LSW preferred)
- 2+ years of social services or case management experience
- Strong interpersonal and communication skills
- Ability to assess needs, develop plans, and monitor outcomes
- Knowledge of community resources and referral networks
- Proficiency with case management software and data entry
- Commitment to cultural competency and client-centered practice
