Join Bridgeview Community Services as a Licensed Social Worker in Chicago, IL. Provide client-centered support to individuals and families, coordinating care across services to promote wellbeing and independence. Responsibility
- Conduct comprehensive biopsychosocial assessments and develop person-centered service plans.
- Provide counseling, crisis intervention, and ongoing case management.
- Coordinate referrals and connect clients with external resources and community partners.
- Advocate for clients within schools, healthcare, housing, and social service systems.
- Maintain accurate documentation in electronic medical records and program databases.
- Collaborate with physicians, therapists, and other professionals to ensure integrated care.
- Support discharge planning, progress monitoring, and family engagement.
Qualification
- Master's degree in Social Work (MSW) from an accredited program.
- Current LCSW licensure in Illinois or eligibility to obtain licensure within 6 months.
- Minimum 2-4 years of relevant experience in clinical, child welfare, or community-based settings.
- Strong communication, cultural humility, and trauma-informed care skills.
- Proficiency with case management principles and electronic health/records systems.
- Ability to work independently and within a multidisciplinary team; excellent organizational skills.
- Valid driver’s license and reliable transportation for community-based duties (if required).
