HopeBridge Community Services is seeking a motivated Licensed Social Worker to join our multidisciplinary team in Denver. This role focuses on supporting individuals and families through case management, assessment, and advocacy. You will collaborate with clinicians, educators, and community partners to connect clients with essential services and resources.

As part of our team, you will:
- Provide comprehensive biopsychosocial assessments and develop client-centered service plans.
- Coordinate with housing, financial assistance, healthcare, and social services to meet client needs.
- Deliver crisis intervention and short-term support as needed.
- Advocate for clients with schools, courts, housing agencies, and other partners.
- Document case notes in the electronic health record (EHR) system with accuracy and timeliness.
- Collaborate with a multidisciplinary team to monitor progress and adjust plans to achieve measurable outcomes.
- Promote strengths-based, culturally responsive care and maintain client confidentiality in all interactions.
Responsibility
- Conduct comprehensive biopsychosocial assessments and develop person-centered service plans.
- Coordinate with community resources to connect clients with housing, financial aid, childcare, and healthcare.
- Provide crisis intervention and on-call support when needed.
- Advocate on behalf of clients with schools, courts, and social service agencies.
- Document case notes and treatment plans in the EHR with accuracy and compliance.
- Collaborate with physicians, therapists, and other professionals to ensure coordinated care.
- Maintain up-to-date knowledge of community resources and policy changes.
- Engage families and individuals in goal setting and progress review.
Qualification
- Master's degree in Social Work (MSW) from an accredited program.
- Current active Licensure as a Social Worker (LCSW or LMSW) in Colorado, or eligibility for licensure.
- Minimum of 2 years of direct client services experience in community-based or clinical settings.
- Strong assessment, case management, and crisis intervention skills.
- Knowledge of local social services and resources in Denver and surrounding areas.
- Excellent written and verbal communication; experience with EHR and documentation systems.
- Ability to work collaboratively in a multidisciplinary team and maintain professional boundaries.
- Bilingual (English/Spanish) preferred; demonstrated cultural competence and empathy.
