Job description

HopeBridge Community Services is seeking a motivated Licensed Social Worker to join our multidisciplinary team in Denver. As part of our team, you will:

  • Provide comprehensive biopsychosocial assessments and develop client-centered service plans.
  • Coordinate with housing, financial assistance, healthcare, and social services to meet client needs.
  • Deliver crisis intervention and short-term support as needed.
  • Advocate for clients with schools, courts, housing agencies, and other partners.
  • Document case notes in the electronic health record (EHR) system with accuracy and timeliness.

Responsibility
