Responsibility
- Conduct comprehensive psychosocial assessments to identify needs, strengths, and risk factors.
- Develop and implement individualized care plans in collaboration with clients, families, and multidisciplinary teams.
- Provide crisis intervention and ongoing counseling to support mental health and safety.
- Coordinate with community resources, housing, healthcare, and social services to secure stable services for clients.
- Advocate for clients to access benefits, services, and legal rights; maintain accurate documentation and case notes.
- Monitor progress, adjust plans as needed, and report outcomes to stakeholders.
- Engage in outreach and engagement activities to connect underserved populations with services.
- Ensure ethical practice, confidentiality, and adherence to relevant laws and agency policies.
Qualification
- Master's degree in Social Work (MSW) from an accredited program; active LCSW licensure preferred.
- Minimum of 2 years of applicable social services experience in healthcare, community mental health, or child and family services.
- Strong assessment, counseling, and case management skills with trauma-informed care approach.
- Excellent communication, advocacy, and stakeholder collaboration abilities.
- Ability to work independently and as part of a multidisciplinary team in high-demand environments.
- Proficiency with electronic health records (EHR) and client data documentation; strict confidentiality practices.
- Cultural humility and commitment to anti-racist, equity-driven practice.
- Valid driver's license and willingness to travel within the service area as needed.
