Join Northstar Community Services, a leading nonprofit focused on empowering families and individuals in the Portland metro area. We are seeking a compassionate Licensed Social Worker (LCSW/LMSW) to join our multidisciplinary team in a full-time capacity.
In this role, you will conduct client assessments, develop and monitor person-centered service plans, coordinate access to housing, employment, mental health resources, and advocate for clients across partner networks. You will also provide crisis intervention as needed and maintain precise documentation in our electronic health record system. Responsibility
- Conduct comprehensive biopsychosocial assessments and risk evaluations for adults and families.
- Develop, implement, and monitor individualized service plans to promote stability and independence.
- Coordinate access to housing, employment, child welfare, mental health, and social services resources.
- Provide crisis intervention, safety planning, and short-term therapeutic support within licensure scope.
- Advocate for clients with systems and community partners to remove barriers to services.
- Document case notes, progress, and outcomes accurately in the EHR and ensure confidentiality per policy.
- Collaborate with an interdisciplinary team to monitor outcomes and contribute to program development and quality improvement initiatives.
Qualification
- Master of Social Work (MSW) from an accredited program; current state license (LMSW/LCSW) in Oregon or eligibility to obtain by start date.
- Minimum 2 years of post-graduate experience in case management or clinical social work, preferably in public health, behavioral health, or child/family services.
- Strong knowledge of trauma-informed care, motivational interviewing, crisis intervention, and evidence-based practices.
- Excellent written and verbal communication skills; proficient with EMR systems and documentation standards.
- Ability to work independently and in a team while managing multiple client cases with sensitivity and professionalism.
- Familiarity with Portland metro area resources and a network of community partners; culturally competent with diverse populations.
- Bilingual abilities (English plus Spanish or another language) are a plus; willingness to travel within the service area as needed.
